Our Community Health Improvement Plan (CHIP) outlines our current priorities. The CHIP is informed by regional Community Health Needs Assessments and feedback from members of the Greater Sullivan County Public Health Advisory Council. The identified priorities are intended to help achieve our goal of understanding the health and safety needs of Greater Sullivan County, engage the community in developing and implementing a comprehensive approach to improving population health outcomes, and advocate for new and existing resources to advance health.
